The Community Learning Disability Team is a team of health professionals who can help adults with a learning disability to stay healthy.
Service Description
Community Learning Disability Teams are made up of people who can find out about your health needs.
They can help you to feel good.
They will help any adult who has a learning disability.
You can ask for help from the team yourself.
Someone who knows you well can ask us to help you. This might be:
• Your doctor
• Your social worker
• Your teacher
